(3 contacts) - Expect to be seen within 96 hours of first contact- Initial telephone contact will be placed to assess need and follow-up plan of care. Subsequent visits to be arranged at first visit. Flexible, tailor-made visits to meet families needs and offer more support in a non-time restricted environment. Conducted in the comfort of your home; 1:1 and supportive visits where all your questions will be answered
